College of Medicine Clinical Portfolio
The 12 item Clinical Portfolio provides candidates with a template to document distinction in clinical practice/patient care. Candidates who have substantial accomplishments in patient care should construct a clinical portfolio for inclusion in their Promotion and Tenure Packet. The Clinical Portfolio may be developed prospectively from the time of hire to serve as a complete record of clinical excellence. At the time of entering the promotion/tenure cycle, candidates who will be using clinical excellence as one of their mission areas for promotion/tenure should submit the completed Clinical Portfolio by inserting directly into the promotion packet as item #24, along with a statement from the clinical chief (division chief or department chair). The portfolio is required for all candidates using the “new” COM guidelines (which were approved in 2012) if the major assignment is patient care and is strongly encouraged for all candidates using the previous (“old”) COM guidelines, if patient care is a major portion of the assignment. The “old” COM guidelines may still be selected for the current 2013-14 promotion cycle. Note that the portfolio is limited to 12 pages. Items should be completed thoughtfully and completely. The instructions may be deleted from the completed portfolio. The 2013-14 Templates follow:
